Community Nursing

The MDHSS community nursing team offers a range of services to residents, as well as to many visitors to the Mallacoota and district region. Services include

  • Post Acute Care
  • Palliative Care
  • Clinical Nursing
  • Assessments.

Both home and centre-based care are provided, with the majority of clients receiving care in their own homes.

Home Care Services Program

The Home Care Services Program provides services to people who are frail aged or disabled, and their carers. Home Care Services focus on enhancing independence by providing a range of services to meet client needs:

  • home help
  • personal care
  • property maintenance
  • respite care
  • meals on wheels.

Community Aged Care Packages

The Community Aged Care Package (CACP) Program assists people living in the community who have been assessed by the Aged Care Assessment Service (ACAS) as eligible to receive low-level hostel care in the home. Clients receiving a CACP have complex care needs over and above the mainstream Home and Community Care (HACC) Program Services.

MDHSS is funded to provide 10 Community Aged Care Packages. Services available to CACP recipients include

  • case management
  • 24-hour emergency on call contact person
  • home care services
  • Home Help, personal care, property maintenance, respite care and delivered meals
  • allied health services socialisation and transport services.

National Respite for Carers Program (NRCP)

MDHSS provides respite services for carers of

  • people with dementia
  • people with dementia and challenging behaviour
  • frail older Australians (65 years and over, or 50 years and over for Indigenous Australians)
  • younger people (under 65) with moderate, severe or profound disabilities who are living at home
  • people with a terminal illness in need of palliative care.

This enables the needs of carers in the Mallacoota and district community to be met without the person being cared for having to leave the community. The aim of the program is to contribute to the support and maintenance of caring relationships between carers and their dependent family members or friends. NRCP facilitates access by carers to information, respite and other support, or assistance appropriate to their individual needs and circumstances, and those of the people for whom they care.

Rural Allied Fly-in Services

The Australian Department of Health and Ageing provides funding for flights to Mallacoota for visiting specialists. These services are a critical component of the diversity of health resources available to MDHSS as a remote community. Services offered once per month by the Rural allied fly-in service are

  • continence nurse
  • speech therapist
  • podiatrist.

Allied Health

MDHSS provides regular Allied Health services to the whole community. In addition to the rural allied fly-in services outlined above, a range of other services are provided by visiting or local specialists. The full range of Allied Services provided includes

Physiotherapy and podiatry services experience the highest demand. These, along with the dietician services, are described in more detail below. For more information on any of the Allied Health services, you can contact MDHSS on 03 5158 0243 between 8.30am and 5pm, Monday to Friday.

The Physiotherapist operates at the centre two days per week and undertakes

  • client consultations and assessments
  • development and monitoring of individual client exercise programs
  • assistance with the delivery of relevant health promotion activities.

Podiatry services are provided monthly at the centre, with some home visits scheduled for people who are unable to access the centre due to frailty or disability. There is a high demand for this service and at times, MDHSS manages a client waiting list.

Dietician services continue to be offered monthly to clients at the centre and in their own home. The dietitian assists people through assessment, diagnosis, treatment and nutritional counselling.

Palliative Care / Post Acute Care

MDHSS provides a coordinated team approach to deliver palliative care services to clients who choose to die at home. Comprehensive services are offered to the palliative client and their carers, which include

  • community nursing
  • allied health
  • personal care
  • home-based services
  • counselling
  • financial services
  • welfare
  • respite care
  • 24 hour short-term respite care
  • provision of equipment

Post Acute Care (PAC) provides short-term planned care for clients and their carers following discharge from a public hospital. The PAC Program is a preventative program that provides a range of health services in the home and at MDHSS. The aim of the program is to diminish the risk of a person being re-admitted to hospital.

Family Counselling

Family counselling is available weekly, Monday to Thursday at MDHSS. Counselling services aim to strengthen the capacities of families and individuals in the community, and minimise those at risk. The service provides a mix of counselling, practical assistance, service coordination, case advocacy and social support to people living in the region.

Dental Clinic

MDHSS operates a private dental clinic and provides a full range of Private, DVA and Teen Dental services including regular dental checkups, dental fillings, dentures, crowns, bridges, root canal and cosmetic dentistry. Emergency dental services can also be accessed on treatment days. HiCaps and EFTPOS facilities are available.

Volunteer Transport Service

MDHSS is funded by the Home and Community Care Program (HACC) for volunteer coordination to provide a volunteer based community transport service. The volunteer transport service assists eligible clients to attend their nearest specialist medical appointments. Eligible clients are frail aged, people with a disability and their carers who do not have family or friends to transport them to their specialist medical appointments.

Maternal and Child Health

The Maternal and Child Health Nurse visits the centre bi-weekly and is an integral part of MDHSS services. This service provides

  • family support
  • health promotion and education
  • early identification and intervention for health concerns.

The Shed

The Shed was established in 2004, through a grant from the Rural Health Strategies Branch, Department of Health and Ageing. The Shed is both a community building project and a project that focuses on the health and wellbeing of people in the community. The project is aimed at people of all ages and provides a centralised meeting space, providing opportunities to socialise, engage in structured activities, share and gain skills and knowledge, and gain access to relevant services and information available within the community.
